How to Change your Provider?

Seniors Community Care will visit you at your home to discuss your what your needs are, what’s important to you and provide you with the information you need to make the most from your Home Care Package.

We will work with your existing provider and the department to transfer your Home Care Package, and ensure no disruption to your services.
